Including QoL, because of the nature that they are all qualitative. Other examples are customer satisfaction, performance evaluation, market research. It takes 3 forms, telephone interview, in-person interview, questionare.

Questionare allows anonymity, free text, and cheap. Cons: poor response rate if there is no incentives.
