1, This is a one-arm study. You may need a control arm.
Without one, this may not be called RCT
2, I don't think ANOVA is applicable here. I don't know what to use to analyse your data. Linear regression? Better speak to statistician
3, This is not a survival study for sure. Survival study is misnomer. It is a study of non-reccuring event, such as life/death. It is not limited to life/death, as long as it is non-recurring. Yours is not.
4 a blind scorer will not solve bias in your test. It is the nature of this type of study. Make sure your questionaire is a quantatative one. Sounds like yours is. If you can find way to blind subjects, that will be ideal.
5 this is more of a QoL study.
6 again get a control group.
